These choices were not always available for cataract patients. In the past, cataract surgery only involved monofocal lenses, which could only focus on objects near or far, but could not adjust to accommodate varying distances. These patients still had to rely on glasses or contact lenses after surgery in order to see clearly at all distances.

Monofocal IOLs.

This group of lenses has a single point of focus. Commonly, patients choose monofocal lenses with an optical target for “distance vision”. Within an arm’s reach, reading glasses (available OTC) are often needed for clarity, especially for reading. Alternatively, patients may choose to have targeted “monovision” or “blended vision”. One IOL (generally the dominant eye) is targeted for distance focus. The other IOL (non-dominant eye) is targeted for near or intermediate vision. Thus, patients may have less reliance on glasses. For cataract surgery, monofocal lenses are covered by insurance (further choices below are not).

Toric IOLs.

Toric IOLs are similar to monofocal IOLs, but are also intended to reduce or eliminate astigmatism. For patients with astigmatism, toric IOLs may provide the sharpest vision result. As with monofocal lenses, toric IOLs can be used to target “distance vision” in each eye (with expected need for reading glasses) or for a target of “monovision”.

Accommodating IOLs.

TheCrystalens is the only lens in the United States approved as an accommodating lens. It has the potential ability to flex within the eye, much like a natural, younger lens. The Crystalens may increase the chance of glare. Not all patients with the Crystalens implant achieve an accommodative result (in which case the lens acts as a monofocal), and most have a limited range of accommodation (still need glasses for fine print). The majority of patients with Crystalens enjoy an extended range of vision while averting some risks of multifocal lenses.

Note: Mile High Eye Institute is happy to provide the newest technology in accommodating lens implants. The Trulign lens implant combines the technologies of the Crystalens and the Toric IOL, which not only reduces dependence on reading glasses, but also corrects astigmatism at the same time.
